Thunderclap triad → subarachnoid haemorrhage until proven otherwise. CT first; LP only ≥12 h after onset if CT is negative — xanthochromia needs time. A tempts you because LP feels decisive. Examiners write distractors from real cognitive errors. Examora names yours.
